Hip Arthroscopy Technique for Acetabular Osteochondral Loose Bodies after Combined Lower Extremity Trauma

The postoperative rehabilitation of acetabular fractures after fixation may be complicated by osteochondral loose bodies in the hip joint and resultant mechanical or instability symptoms. There are multiple surgical options to address these lesions, however, arthroscopic removal exposes the patient to a smaller incision and lower risk of complication when performed appropriately. As these high-energy injuries commonly occur with ipsilateral lower extremity osseous and soft tissue injury, traction for arthroscopy may be limited. We describe a case of an acetabular fracture and ipsilateral open tibia/fibula fracture with osteochondral loose bodies in the hip joint inhibiting physical therapy potential, treated successfully with arthroscopic removal using an adjusted skeletal traction method.
