The management of massive rotator cuff tears in healthy active patients without evidence of glenohumeral arthritis can present a treatment challenge. Mixed results have been reported in the literature for different treatment options, including arthroscopic and open debridement, partial repair, muscle transfers, and arthroplasty. We present an easily reproducible surgical technique using porcine dermal xenograft as an interposition graft for the management of massive, otherwise irreparable rotator cuff tears. © 2012 by Lippincott Williams & Wilkins.
