Effect of amlodipine on mode of death in advanced heart failure

We studied 1,153 patients with severe chronic heart failure from the Prospective Randomized Amlodipine Survival Evaluation to determine the effect of amlodipine on mode of death. When mortality was evaluated based on the cause of heart failure, we observed a significant reduction in all-cause and cardiovascular mortality, in nonischemic patients only, who were treated with amlodipine. Because this finding was unexpected, however, further trials that evaluate cause-specific mortality in heart failure should be done to determine the safety of amlodipine in this population.
