We evaluated the effectiveness and costs by giving monitored prophylaxis to low-risk tuberculin reactors aged 35, 50, or 70 years. Compared with no prophylaxis, we found that isoniazid prophylaxis increased the probability of survival at 1 year and for all subsequent years. If all low-risk tuberculin reactors over the age of 35 in the United States were given prophylaxis, 35,000 lives could be saved and health expenditures reduced by more than $2 billion.
