Bone graft options and harvesting techniques

Spinal arthrodesis is performed for a variety of congenital, degenerative, traumatic, and neoplastic disorders. A number of operative approaches, surgical techniques, and types of bone graft have been evaluated for consistency of fusion and complication rate in spinal fusion applications. The authors review the biologic factors influencing spinal arthrodesis, the fusion rates for autograft and allograft bone, and the complications associated with autogenous bone graft harvest. The surgical technique for iliac crest and fibular bone graft harvest is described, with an emphasis on complication reduction. Autograft continues to be indicated for most patients with spinal arthrodesis despite the potential for donor site morbidity. Proper surgical technique and an awareness of the range of potential complications can reduce their occurrence.
