Idiopathic glossopharyngeal neuralgia is rare. The diagnosis is not difficult and treatment is only surgical. The cervical approach is the best for the condition. We have had no recurrences so far during 1 to 2 years followup. Excision of tonsils and/or elongated styloid process gives only temporary relief and a long followup proves that neurectomy is the solution to the problem. Wilson's transtonsillar approach is an inadequate procedure. © 1978 Association of Otolaryngologists of India.
