Robotic Bariatric Surgery: An Update

Purpose of Review: This article will provide a review of the most recent literature regarding the use of robotics in bariatric surgery. Recent Findings: The use of robotics in bariatric surgery, while still debated, is on the rise. The appeal of this platform includes improved surgeon ergonomics, visualization, dexterity as well as decreased dependence on surgical assistance for exposure and camera use. This must be balanced against learning curve, increased operative time and cost as well as safety profile. There are numerous retrospective studies that have been performed recently in an attempt to better define the utility of the robot in bariatric surgery, especially when compared to the gold standard of laparoscopy. While many studies continue to show increased operative time and cost, patient outcomes are comparable to laparoscopy. Newer studies are showing that cost and operative time can be significantly improved once the learning curve is overcome. Additionally, the field of robotics continues to evolve with advances in current platforms and the development of new ones. Summary: Despite limited use in bariatric surgery, robotic surgery has the potential to be of great use to the bariatric surgeon, especially in the realm of revisional surgery and when operating on patients with a higher body mass index. As its use becomes more widespread and continues to evolve, the disadvantages like operative time and cost are likely to dissipate. Further studies that consider factors like technique and level of operative experience are certainly warranted.
