Posterior shoulder instability is an uncommon isolated finding that can result in functionally limiting shoulder pain and weakness. While arthroscopy is the mainstay of current management, an open approach may be utilized, either alone or in combination with arthroscopy, to address recurrent pathology or challenging anatomy. In this chapter, we describe the background, diagnosis, treatment, and outcomes for posterior instability of the glenohumeral joint treated with open surgical repair.
