Low-risk differentiated thyroid cancers (DTC) are defined as tumors that are less than 4 cm in size without evidence of extrathyroidal extension or nodal disease (in patients without a family history of thyroid cancer or personal history of radiation to the head and neck). Treatment options for low-risk DTC can include total thyroidectomy, ipsilateral thyroid lobectomy, or active surveillance. In this chapter, we discuss criteria for determining the optimal treatment strategies for patients with low-risk DTC, using clinical and radiographic features.
