Frequency and location of benign prostatic glands in the surgical margin of 147 radical retropubic prostatectomies for carcinoma

Introduction and Objectives : Three dimensional studies of whole mount total prostatectory specimens have revealed the presense of benign prostatic tissue as the surgical margins . The study was undertaken to evaluatte the frequqncy and location of benign prostace tissue at the surgical margin and correlate with clinical stage. Methods ; 147 radical retropubic prostatectomy specimens were fixed in formal in and sect ioned at 2.5 nm intprvals after the surgica1 margins were col or coded according to right and left half, processed as whole mounts, and 3 dimensionally reconstructed. Results; In 36 of 147 specimens, benign glands were found mostly in the posterior and posteroldteral regions, in 5 in the anterior resection maigin, in several in both the anterior and posterior" margins, and in 7 in the same region as the carcinoma. In 15, the tumors were organ confined. Pathological and clinical stages were as follows : Clinical Stage Pathologicai Stage x 2a 2b 2c 3a 3b 3c T1c 10 1 8 2 2 2 T2a 1 1 2 T2b 2 1 1 2 T2c 2 T3 13 2 1 13 4 3 Conclusions : This surpriziing finding may be of clinicai significance as it may be mi s interpreted as capsular penetration. or "positive margin" Persistence of elevated PSA after prostatectomy may be due to residual benign prostate tissue.
