A case of massive pulmonary embolism complicated by acute cor pulmonale and shock is presented. The IV administration of recombinant tissue-type plasminogen activator (rt-PA) was associated with prompt reversal of clinical, ECG, and nuclea radiographic findings. The role of thrombolytic therapy with rt-PA for massive pulmonary embolism associated with hemodynamic decompensation deserves further investigation.
