Case report: Catatonia and neuroleptic malignant syndrome in the nursing home

Because many illnesses present with fever and confusion in the long- term care setting, neuroleptic malignant syndrome (NMS) in the elderly is a challenging, yet uncommon, diagnosis. Catatonia is implicated as a predisposing factor for the development of NMS and may be associated with major depression in cognitively impaired elderly individuals. A severe variant of catatonia, malignant catatonia, is clinically similar to NMS and may be related pathophysiologically. The authors present a case of rapid cognitive decline in an elderly woman that illustrates the association between catatonia and NMS.
