The augmented anastomotic urethroplasty is a straightforward technique for managing urethral strictures that fall in the gray zone between those that can be managed by stricture excision and primary anastomosis alone and those that require onlay substitution urethroplasty. By combining the principles of both techniques, tight urethral strictures can be excised and adjacent spongiofibrotic urethra onlayed with a graft or flap such that the amount of urethral shortening and the amount of nonurethral tissue incorporated into the repair are minimized. The results are good, with a stricture-free rate <90% and minimal complications. Augmented anastomotic urethroplasty is an important addition to the urethroplasty armamentarium.
