Future trends in chronic disability and institutionalization: implications for long-term care needs.

Demographic changes (including a healthier and aging population), trends in chronic morbidity and disability, and changes in active life expectancy have affected trends in long-term care delivery in the United States. Managed care plans face particular challenges, including creation of mix-based reimbursements and better integration of geriatric and other specialty services.
