This chapter describes the conceptual framework of surgical management of rectal cancer. The anatomic basis for the technical components of proctectomy is discussed. The importance of multidisciplinary care and decision-making is emphasized. A variety of approaches are reviewed, including open operations and minimally invasive techniques, including laparoscopy, robotics, and taTME. The impact of restorative proctectomy on function and the importance of the neorectal reservoir are highlighted. Locally advanced rectal cancer and its complexity and attendant challenges are enumerated.
