Endovenous thermal ablations and chemical ablations with sclerotherapy have become the mainstays of treatment for superficial venous reflux disease. Despite these advances, there is a role for stripping and open phlebectomy in the setting of advanced venous disease with tortuous varicosities, history of thrombophlebitis, and aneurysmal vein segments. This chapter explains proper patient selection for open surgical management and its techniques.
