Breast radiologists are frequently consulted for evaluation of patients with concerns related to skin changes of the breast. A broad spectrum of dermatologic conditions can manifest on the skin of the breast, the chest wall, and the axilla, including those with benign, malignant, infectious, and inflammatory causes. These conditions often pose a diagnostic challenge for breast radiologists as they may be associated with nonspecific or absent imaging findings. Moreover, such cases may fall outside the traditional scope of breast imaging training, potentially contributing to diagnostic uncertainty or delays in appropriate management. To effectively evaluate these patients, breast radiologists must possess a foundational understanding of skin anatomy as well as familiarity with the clinical morphology and physical examination characteristics of dermatologic conditions involving the breast. Integrating clinical history, physical findings, and imaging features is essential to appropriately determine when a skin punch biopsy is indicated. This review is intended to serve as a practical dermatologic primer for breast radiologists, providing a structured overview of benign and malignant cutaneous conditions commonly seen in breast imaging practice and guiding appropriate clinical triage and decision making. ©RSNA, 2026 Supplemental material is available for this article.
