Spreading awareness to the hospitalist group at DRH regarding early discharge initiative. Assess the metrics "discharge order by 9 am" during a 6 month period with periodic sharing of performance of the group. Recognize the top performers for this metric and share the tricks for improvement in this metrics and evaluate if it would translate to improvement in early discharge and LOS.
Weekly review of long length of stay patients with hospital medicine leadership to brainstorm creative solutions.
Afternoon discharge huddle with case-manager, charge nurse and provider to improve early discharge.
Multi-disciplinary team and performance services collaborated to identify areas of improvement in workflow processes to improve MDR and geographic patient assignment.
