Antiplatelet therapy is the first-line antithrombotic drug treatment for secondary ischemic stroke prevention in most situations, with the notable exception of atrial fibrillation. Drugs other than aspirin offer a small additional benefit. Anti-platelet agents have only a modest role to play in the treatment of acute ischemic stroke and are little value for primary stroke prevention.
