A 21-year-old woman presented with a large, well-circumscribed breast mass. Magnetic resonance imaging (MRI) showed multiple cleftlike, fluid-filled spaces within the mass, a feature described as characteristic of phyllodes tumors. Pathologic examination showed a benign giant fibroadenoma with an intracanalicular pattern with dilated, fluid-filled clefts. This variant of the MRI appearance of giant fibroadenoma and potential overlap with phyllodes tumor is described.
