Successful procurement and optimal cardiac preservation are the cornerstones of any successful heart transplant. This chapter reviews the technical and logistical challenges involved in the procurement of the donor heart and the variety of solutions and techniques commonly used for organ preservation and assessment. The nature of organ transplantation mandates continuous communication with several parties. Donor heart procurement is performed via median sternotomy. Donation after circulatory death requires that a patient who is not brain dead have cessation of circulation prior to organ procurement. This is defined as absent arterial pulsation and mechanical asystole. All heart transplantation requires some period of cold ischemia and ex-situ transportation. There are over 100 different preservation solutions available for the heart and only 55 are standardized. These solutions can be broadly categorized into intracellular solutions versus extracellular solutions based on their electrolyte composition.
