Treatment of Perioperative Ischemia, Infarction, and Ventricular Failure in Cardiac Surgery

An increasing number of high-risk patients are undergoing cardiac surgery. Despite this trend, advances in surgical techniques, myocardial preservation, anesthesia, and perioperative care have resulted in reduced morbidity and mortality. Positive outcomes require timely recognition and management of perioperative myocardial ischemic injury and ventricular dysfunction. These sentinel events can have catastrophic consequences if mismanaged, and they are determinants of short- and long-term outcomes. This chapter discusses the etiologies of myocardial ischemia and ventricular dysfunction, as well as management that includes pharmacologic, catheter-based, and mechanical treatments.
