Telemedicine Visits After Hospital Discharge in Children With Medical Complexity

Children with medical complexity utilize health care at increased rates, and their caregivers feel unprepared to care for their child at home. Through a pretest/posttest design, this quality improvement project explored using telemedicine following hospital discharge. Twelve caregivers reported satisfaction with the telemedicine intervention and rated their confidence providing medical care to their child above the projected goal. There was a statistically significant decrease in unplanned hospital readmissions following the implementation. Overall, telemedicine in children with medical complexity serves to bridge the care they receive in the hospital as their caregivers establish a routine at home.
