Emerging Technology: Robotic Inguinal Hernia Repair

Inguinal hernia repair is one of the most common operations performed by general surgeons worldwide. Several techniques for open and laparoscopic repair of inguinal hernias have been well described and are common in general surgical practice. These techniques have resulted in low morbidity and recurrence rates; however, surgeons continue to develop alternative techniques or technologies to improve outcomes of inguinal hernia repairs. Robotic surgery, which has traditionally been used most in gynecologic and urologic surgery, has undergone tremendous growth in the field of general surgery. General surgeons now have increasing access to robotic surgery and are exploring its use in all fields of general surgery, including hernia surgery. The rationale for using robotic surgery for inguinal hernia repair is similar to its use in other fields of surgery, including improved visualization and dexterity, surgeon comfort, and possible improved outcomes. Currently robotic inguinal hernia repair is only being done clinically using the transabdominal preperitoneal (TAPP) approach, but much research is ongoing to adapting this technology to the totally extraperitoneal (TEP) approach. Many surgeons are starting to perform robotic inguinal hernias; however, to date very few studies addresses its efficacy. Further evaluation and follow-up are needed to determine whether robotic inguinal hernia repair is a viable alternative to open and laparoscopic techniques for repair of inguinal hernias.
