Passenger lymphocyte syndrome (PLS) is an immune-mediated complication that can occur following solid organ transplantation, resulting from the transfer of donor-derived lymphocytes that produce alloantibodies targeting recipient antigens. We report a case series of PLS presenting as severe refractory thrombocytopenia in multiple recipients of solid organ transplantation from a single donor.
