The risk of renal failure and mortality related to it after biliary surgery for obstructive jaundice have been evaluated. Renal failure has been differentiated in functional and organic forms while predictivity of clinical and biochemical data was analysed. Among the patients considered in the present study, 21,6% developed various degree of renal impairment, and 6 patients died for tubular necrosis (2 patients) or hepatorenal syndrome (4 patients), representing 79% of post-operative mortality. Long duration of jaundice, high level of serum bilirubine and low protombine activity resulted significantly related with renal failure. Renal failure after surgical treatment of biliary obstruction appears closely dependent on hepatic insufficiency and the cause of death is (hepatic) rather than (uremic).
