Transcorporal placement of an AUS cuff is a simple and useful technique for those complex cases of artificial sphincter implantation that require placement of the cuff in a more distal location than usual. By minimizing the dissection of the urethra and incorporating a buttress of tunica albuginea tissue around the urethra, the risk of injury to and/or erosion of the urethra is lessened, and girth is added to the urethra for a better cuff fit. Continence rates are excellent, complications are minimal, and patients are generally very satisfied. Because of the risk of erectile dysfunction, this procedure is probably best reserved for men who are already impotent. Considering the salvage nature of the patients who are implanted, the results should be considered exceptional.
