1. Develop a nurse-driven telehospice program to provide high quality services to home hospice patients in rural/undeserved counties in Western North Carolina
2. Improve patient/family satisfaction and health outcomes by implementing a care management portal to be used in conjunction with the telehospice program
3. Increase nursing leadership and inter professional collaboration within a virtual telehealth system
