Vascularized composite allotransplantation is a field in its early development. This chapter describes the latest classification scheme applied to composite grafts and acute cellular rejection is amply demonstrated with high quality images. The differential diagnosis of mimics of rejection in the skin and a brief account of the immunologic mediators of rejection and future directions are included. © 2011 Springer-Verlag Berlin Heidelberg.
