Complex primary total knee arthroplasty: Management of extra-articular deformity

Total knee arthroplasty (TKA) in the setting of extra-articular deformity is a challenging problem, on scale with revision TKA. Exhaustive preoperative planning is imperative for a successful outcome, and to determine if intra-articular correction is possible or if extra-articular correction is necessary. The patient should be warned of increased risks and decreased outcomes as well as screened for indolent infection. The use of computer assistance, especially in the setting of femoral deformity, is extremely valuable. Options to increase the constraint of the TKA should be readily available.
