Clinical trials of mechanical circulatory support devices evolved from simple studies evaluating the efficacy of the device in a few patients to large-scale clinical trials with composite endpoints including quality of life, improvements in functional capacity, and freedom from major complications in addition to mortality. Since the mid-1980s, there has also been a change in pump design from paracorporeal pumps to intracorporeal configurations and from pulsatile to continuous-flow designs. In this chapter, we review the major clinical trials that have been performed that have propelled the field forward for both bridge to transplantation and destination therapy.
