Opioid Tapering for Acute on Chronic Non-cancer Pain

Patients with chronic non-cancer pain on opioid therapy can present with acute pain, most often after surgery. Escalations in opioid dosing often occur in these patients, and weaning of opioids after the acute event can be challenging due to fear of worsening pain, concern for withdrawal, and patient reluctance. Little research currently exists on optimal ways to taper opioids in these instances. Here we offer guidelines to help clinicians perform this challenging task.
