Optical coherence tomography (OCT) has revolutionized the field of ophthalmology, providing noninvasive, high-resolution imaging of the retinal layers. In recent years, use of OCT in the operating room, or intraoperative OCT (iOCT), has emerged as a valuable tool in retinal surgery, enabling real-time visualization and guidance during surgical procedures. This chapter explores the principles, appli-cations, and benefits of iOCT in various retinal surgeries. Different cases are used as examples to discuss the impact of iOCT on intraoperative visualization and surgical decision-making. Additionally, the chapter addresses the current challenges and future directions for the integration of iOCT into routine retinal surgical practice.
