The direct brow lift or direct brow-pasty is an effective surgery to treat brow ptosis. The direct brow lift offers optimal functional results due to the tissue resection that is in immediate proximity of the eyebrows. The main shortcomings of this procedure are the visible post-operative scar, unnatural brow shape, and potential forehead hypoesthesia, though these risks can be mitigated with appropriate patient selection and good technique. In this chapter, the authors describe direct brow lift surgery including a technique of a medial chevron-shaped supraciliary incision that helps prevent an irregular brow contour.
