This article describes different novel approaches to individualizing care at the patient and family member level in the intensive care unit (ICU) and beyond. The first example discussed is using web-based decision support that displays information individualized to patients in the context of prolonged mechanical ventilation. The second example highlights approaches to addressing post-discharge psychological distress using mobile apps. The final example outlines a mobile app system that integrates both physician and family member perspectives while also enhancing the delivery of personalized palliative care. Successful attributes of these interventions are noted, along with the challenges that represent targets for future improvement.
