CASE: A 77-year-old woman with chronic pelvic discontinuity and type IIIB acetabular bone loss after right hip hemiarthroplasty underwent acetabular reconstruction using a custom triflange acetabular component. A mixed-reality image-based navigation system was used intraoperatively to guide bone loss assessment and component placement. As far as we know, this is the first report of this technology in custom triflange reconstructions. CONCLUSION: Reconstruction of acetabular bone loss is challenging. Mixed-reality systems may improve intraoperative decision making, surgical efficiency, and outcomes in this vulnerable population.
