Several new fentanyl derivatives are currently under evaluation in clinical trials and will eventually be available for clinical use. This article reviews new concepts in opioid pharmacokinetics and discusses several of these new opioids. Novel routes of opioid administration are explored in addition to mixed agonist-antagonist drugs and newer narcotic antagonists.
