Basic concepts in the design of one clinical data base are presented that are used in three health care facilities at the Duke University Medical Center. The programs are written in GEMISCH, and all components of the medical record are dealt with. The use of a modular design and a dictionary provide a highly flexible system with considerable freedom of entry and print formats. Both direct, interactive data entry and entry by a data terminal operator are supported. The amount and type of data retained in the on-line file is controlled by the user. This working clinical data base system offers much potential in solving some of the informational problems of health care delivery.
