Living donor kidney transplants constitute between 10% and 50% of annual kidney transplant volume worldwide. This chapter presents the history of living donation through the modern-day approach to living donation, with a focus on evolving paradigms of what constitutes an acceptable donor, approaches to immunologic and biological incompatibility, and nonmedical considerations in the living donor workup and recovery.
